Born in Radiation
When the Venom symbiote was first handed over to S.H.I.E.L.D, Otto Octavius was put in charge of experimenting on it. He used a number of different ways to try and discover what it really is. This led him to using radiation on it, to see how the specimen would react. However, it had an unexpected effect, and the symbiote split into two. One retained its black colour, while the other one developed a blood-red colour. Days later, the new symbiote, having been dormant, suddenly came alive and began causing destruction, breaking free of its cell. It was nicknamed "Carnage" and after several hours of attempting to capture it, S.H.I.E.L.D succeeded. They managed to get it into a small container, frozen and put in an electrified prison.
